Michael T. Cusack, Sr., 53, of Brimfield, passed away at 3:20 p.m., on Wednesday, January 4, 2023, at OSF Saint Francis Medical Center in Peoria.
He was born December 26, 1969, in Peoria, to Ronald and Sandra (Jordan) Cusack.He is survived by two children, Michael (Melanie) T. Cusack, Jr., of Peoria, and Gabriel Cusack, of Edwards; one brother, Randy (Tina) Cusack; and one sister, Veronica (James) Stewart.
He is preceded in death by his mother; and one sister, Rita (Frank) Schuck.
Cremation rites have been accorded, and a Celebration of Life will be held at a later date.
