He was born in Canton, to Donald and Sharon (Ball) VanMiddlesworth. They preceded him in death.
He is survived by his significant other, Monica Clendenny, brother, Tim (Christine) VanMiddlesworth, nephews, Bryan Wolford and Craig VanMiddlesworth, Niece Megan Rosich, and great nieces and nephews, Alicyn, Vince, Ethan, Tannor, and Koltin.
Tom last worked for LG Seeds in Elmwood. He enjoyed hunting, fishing, and mushrooming. He also liked to take road trips. Tom was a gifted woodworking and skilled at working with leather.
Cremation rites will be accorded with Oaks-Hines Funeral Home and Crematory in Canton.
