He was born on June 26, 1954 in Canton, IL, to Clark and Betty Bridgman Formhals. They preceded him in death.
Surviving are his children Rhonda (Brian) Royer of Canton, Timothy (Courtney) Formhals of Carthage, one brother David Formhals of Canton, four grandchildren, Parker, Addison, Ava and Lyla.
Also preceding him in death are two brothers Eldon and Roger.
Dan last worked for SVI as a truck driver and also enjoyed fishing, 4-wheeling and loved his German Shepherd “Baby”.
A celebration of his life will be held on Friday, May 1, 2015 at 11 am at the Evangelical Free Church in Canton with the family receiving friends one hour prior at the church.
Cremation rites have been accorded.
Oaks-Hines Funeral Home in Canton is in charge of arrangements.
Memorials may be made to the Humane Society of Fulton County.
